Effective February 1, 2025, Molina Healthcare of Florida is only servicing Medicaid members in Miami-Dade and Monroe counties.

You should have received information in the mail from your new health plan if you are not located in Miami-Dade and Monroe counties. If not, please call Choice Counseling at (877) 711-3662 (TDD: 866-467-4970) Monday – Friday, 8 am – 7 pm ET.
